Case Manager – Duluth

Work Hours: M-F, 8:00am to 4:30pm and as needed

License/Certification Requirements: Valid MN Driver’s License, current insurance or be insurable.
Summary: Responsible for providing comprehensive Case Management services to children & families who are involved in, or is at risk to be in the Child Welfare system. Work directly with children & parents with the intent to prevent or reverse the removal of children from the home. Provides support services to enhance or ensure the family’s ability to achieve child safety, to teach, and reinforce positive family relationship skills, to promote the well-being and permanency of children in a stable home environment.
Education Requirements and Experience:
Bachelor’s Degree in Social Work (BSW) or other Human Service related field is required, Associate’s Degree in Human Services may be considered, only if currently enrolled in an accredited post-secondary institute, working towards Bachelor’s level degree and degree is obtained within two years.
Minimum requirement is an AA in Human Services field, with case supervision required.
Experience working with Native American children and families is preferred.
Experience in Microsoft Office applications is preferred. Native American preference.
Additional Qualifications and Skills:
Knowledge of the Minnesota Indian Family Preservation Act and the Indian Child Welfare Act is preferred.
Knowledge of Minnesota Statutes as they apply to child maltreatment is preferred.
Knowledge of the Leech Lake Tribal Codes relating to Child Welfare Services is required, or willingness to learn. Familiarity with Title IV-E Tribal/State Agreement is required or willingness to learn.
Familiarity with the Leech Lake Band of Ojibwe Human Services program and surrounding county Social Service agencies is required or willingness to learn.
Good verbal, written communication, and customer service skills are required.
Ability to pass pre-employment drug test, criminal, and Social Services background check is required. Pre-employment drug testing is mandatory for all Leech Lake Band of Ojibwe positions.
Duties/Responsibilities:
Be on call and respond to resolve crisis situations within the community as needed.
Manage a potentially high client case load of families residing within the Leech Lake Reservation boundaries.
Develop a case plan with the families and monitor to ensure goals are achieved.
Provide support and motivation to families in reaching their goals.
Work individually with families on parenting issues and building parenting skills, or provide appropriate referrals. Collaborate with other programs/resources to facilitate family group activities for the purpose of strengthening family structure and build on family values.
Conduct scheduled and unscheduled client home visits.
Arrange for necessary meetings with other resource agencies and provide referrals to ensure the family receives appropriate culturally relevant services.
Coordinates services to work with the family, relatives, and children in resolving family crisis situations.
Attend Case Review Team meetings.
Provide court reports to appropriate individuals and attend court hearing involving the family.
Facilitate supervised visits to mentor and model positive parenting roles.
Provide face-to-face visits with the family on a minimum of twice per month.
Become knowledgeable with the North Star Care for Children.
Assist clients in completing eligibility benefit applications.
Submit accurate targeted case management billing forms ensuring documentation is on file for eligible services.
Maintain client records through the Social Services Information System (SSIS).
Complete service arrangements to authorize payments for support services.
Submit adequate and timely reports to management upon request.
Attend the required trainings listed in the “Required Training” for relative to the job position.
Maintain a respectful working relationship with colleagues, Leech Lake Reservation program employees, count Human Services staff, and other community stakeholders.
Meet deadlines, prioritize workload, and work effectively with minimal supervision.
Attend program activities outside normal working hours as necessary.
Support and maintain sensitivity to Native American culture, lifestyles, and values.
Attend and/or conduct all mandatory meetings and trainings as required. Maintain a high degree of confidentiality and professional ethics at all times.
Travel in and out of the state as needed.
Schedule routine and/or random client drugs screens when appropriate.
Attend regular staff meetings.
Provide appropriate service referrals.
Attend the Basic Social Worker Core training series as presented by the Department of Human Services.
Adhere to policies governing staff conduct.
All other related duties as assigned.
Physical Requirements:
Ability to lift 30 pounds or more and use proper body mechanics.
Be able to sit or stand for extended periods of time.
